The East Wellington Diabetes Team includes Nurses and Dietitians, who provide education and support in group workshops and one-on-one appointments to patients and community residents with:

  • Age 18 +
  • Diagnosis of Prediabetes or any type of Diabetes
  • Insulin starts and adjustments once insulin orders are received

The Team is not insulin pump certified and does not have an ADP approved physician.

The Diabetes Team provides support so you may make healthy choices regarding food, activity and medication.  Our goal is for our patients to gain skills to make informed choices to help prevent long term complication that can come from diabetes.

Referrals:  

East Wellington Family Health Team patients can discuss a referral with their doctor.

Community residents without an EWFHT doctor may self-refer by completing this SELF-REFERRAL FORM and dropping it off at one of our clinics.
